Chronic conditions impact quality of life, employee productivity, and health care costs.
Our health coaches provide support to members and their families with common and complex chronic conditions. Our Chronic Conditions Management program focuses on self-management through knowledge and empowerment. Health coaches assess the members’ level of knowledge about their condition and provide the necessary education and resource information. Candidates are identified through direct member enrollment, utilization or case management referrals, or an analysis of predictive modeling based on claims date. Our programs are accredited by the National Committee for Quality Assurance (NCQA). The managed chronic conditions include, but are not limited to:

Complex Chronic Conditions such as:
